Below you will find our new patient forms. Feel free to print and complete the forms before your first visit with us. While we do not require that you complete this paperwork prior to visiting our practice, this will save you and our staff time when you come in for your appointment.
WHAT TO EXPECT AT SEXTON CHIROPRACTIC
When you come to Sexton Chiropractic for an appointment, you can expect prompt service! Our doctor and staff work hard to make sure every patient feels relaxed and at home when they walk through our doors. Since chiropractic care generally involves more than one visit, our team works to build trusting relationships with every patient. We believe the best way to have a successful patient relationship is to fully explain everything in advance. This way the patient knows exactly what their treatment is going to be and what it involves. The more the patient knows, the more they can relax when they come in for their appointment.
WHAT YOU CAN EXPECT AT YOUR FIRST VISIT
During your first visit to Chiropractic Professionals, you will have a:
Consultation with Dr. Sexton about what is concerning you
Thorough chiropractic, neurological, and orthopedic examination to help locate the cause of the problem
Referral for x-rays if needed
If it is determined that chiropractic care is appropriate for your condition, you will then receive your first treatment.
BUILD ON YOUR RESULTS WITH REGULAR VISITS
After your first visit, you will likely need to follow up with several regular visits to properly correct your condition. Routine visits are generally much shorter than the first one since Dr. Sexton can complete a visit in just a few, highly-focused minutes. Each visit to our office builds on the one before, so your readjustment will be easy with continued visits. So if you miss an appointment or discontinue care too soon, you may also lose the progress you have achieved from previous treatments.
To make an appointment, call us at (515) 287-3993. Once you make an appointment that time will be reserved specifically for you. If, for any reason, you cannot make your appointment, please call us at least 24 hours in advance to reschedule.
We will do everything we can to work with your insurance to help you find your maximum coverage. If you are unsure how your insurance works, or if you have questions about your chiropractic coverage, just give us a call during our regular office hours.

For patients who have little or no chiropractic insurance coverage, a flexible payment program can be arranged
Payment is expected at the time services are rendered.
This includes co-payments and deductibles (amount not covered by your insurance).
